OUR HISTORY
Integrity Home Health Care is a locally owned and certified home care agency that was founded in 2003 by co-owners, Diana “Dee” Kornetti and Sherry Teague. As clinicians, they were unsatisfied with the home care services that were being offered to our community’s senior citizens. They decided that it was time to start a home health organization with a desire to become a prototype of what health care should be – by putting individualized patient care above all else.
The mission of Integrity Home Health Care is to provide professionally coordinated home health services and products, which embody honesty, sincerity and wholeness to our senior population. With this in mind, we have exceeded standards and expectations on a continual basis.
Since its humble conception 7 years ago, Integrity Home Health Care has grown exponentially. With a staff of over 40 highly-skilled professionals, Integrity Home Health Care now offers an expansive array of services. These include in-home Nursing, Physical Therapy, Occupational Therapy, Speech Therapy and Home Health Aide services. The company currently cares for homebound individuals in Marion, Citrus, Sumter, Levy, Alachua and Lake Counties.
HOME HEALTH SERVICES
Integrity Home Health Care strives to be the most knowledgeable, personable, and thorough home health agency in central Florida. Your goals, expectations, and needs are the driving force to the individualized care that we provide.
Home Health Care is an intermittent, clinical service that is provided to homebound individuals with the intent to keep the individuals living as independently as possible. Individuals may receive home health care if they are living in their own home, in an independent living facility, or an assisted living facility.
In order to be eligible for home health care, the Center of Medicare and Medicaid Services requires the following conditions:
1. You must be under the care of a doctor, and you must be receiving services under a plan of care established and reviewed regularly by a doctor.
2. You must need, and a doctor must certify that you need, one or more of the following:
- Intermittent skilled nursing
- Physical therapy
- Speech-language pathology services
- Continued occupational therapy
3. The home health agency caring for you must be approved by Medicare (Medicare-certified).
4. You must be homebound and a doctor must certify that you’re homebound. To be homebound means the following:
- Leaving the home isn’t recommended because of your condition.
- Your condition keeps you from leaving home without help (such as using a wheelchair or walker, needing special transportation, or getting help from another person).
- Leaving home takes a considerable and taxing effort.
A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services. You can still get home health care if you attend adult day care, but you would get the home care services in your home.
There is no co-pay or out-of-pocket expense associated with home health care for Medicare beneficiaries. For individuals with HMOs, please check with your insurance company regarding any out-of-pocket expenses that may be associated.

PRIVATE SERVICES
Home with Integrity is a private service that provides personal and companion care. We have structured Home with Integrity to accommodate the individual needs of each particular client.
Home with Integrity’s caregivers are employed, trained, and certified by Home with Integrity. Clients and family members should also rest assured knowing that these private services are reviewed and monitored by a Registered Nurse Care Coordinator.
